This information will not be shared with any third parties (with the rare exception of the website hosting partner on an as needed basis to troubleshoot any website related issues or other parties who assist the Practice in the operating of this website).

Our website allows you to contact our office via telephone or a secure, HIPAA compliant electronic consultation request form (via our electronic medical record company's website – currently Luminello). To process your request for a free phone consultation, you may be asked to provide us with personal identifying information and/or personal health information. All information collected will be kept confidential as detailed in our Practice's "Notice of Privacy Policies" (detailed copy available upon request).

We do not share your personal health information with third parties for any reason unless:

  • it is required to process your request(s)
  • it is legally required to do so
  • to enforce our site policies
  • protect the rights, property or safety of others and/or our Practice
